Philippines Dossier Requirements

 

  • Home Study report from a licensed adoption agency
  • 3 notarized Reference Letters
  • Application Form
    • Undertaking of the couple under oath
    • Information and Personal Data Application
    • Special Needs Checklist
    • Self-Report Questionnaire (optional)
  • Certified copies of birth certificates for each parent
  • Certified copy of marriage certificate; Divorce decree(s), if applicable
  • Physical and Medical Evaluation Report for each parent
  • Psychological Evaluation Report
  • Written Consent to Adoption by children in the home who are over 10 years of age
  • Character Reference Letters
  • Most Recent Income Tax Return
  • Local Police Clearance
  • USCIS Immigration Approval (I-800A)
  • Letter of Acceptance of the Designated Guardians
  • Adoption Decree of any previously adopted Filipino child in the family

All of the documents above must be notarized. The dossier must also include:

  • Copy of Passports
  • Passport photo of each parent
  • Recent pictures of the parents, family members and home/community
  • Interagency Agreement/ Licenses
  • Post Adoption Report Agreement
