- Certified copy of marriage certificate; Divorce decree(s), if applicable
- Physical and Medical Evaluation Report for each parent
- Psychological Evaluation Report
- Written Consent to Adoption by children in the home who are over 10 years of age
- Character Reference Letters
- Most Recent Income Tax Return
- Local Police Clearance
- USCIS Immigration Approval (I-800A)
- Letter of Acceptance of the Designated Guardians
- Adoption Decree of any previously adopted Filipino child in the family
All of the documents above must be notarized. The dossier must also include:
- Copy of Passports
- Passport photo of each parent
- Recent pictures of the parents, family members and home/community
- Interagency Agreement/ Licenses
- Post Adoption Report Agreement
